Medicinal Leech Therapy (Hirudotherapy) in Equine Medicine

The therapeutic application of medicinal leeches, is an ancient practice seeing a modern resurgence in veterinary medicine. It is primarily employed as a complementary or alternative treatment when traditional therapies fail or yield slow results. The main purposes of this therapy are to treat inflammatory conditions, vascular disorders, and various musculoskeletal or dermatological issues.

Bioactive Chemical Substances

The effectiveness of hirudotherapy stems from the saliva of the leeches, which contains a complex "cocktail" of 60 to over 100 bioactive proteins and enzymes. Key substances include:

  • Hirudin: A potent natural anticoagulant that inhibits thrombin and prevents blood clotting.

  • Calin: Inhibits collagen-induced platelet aggregation and ensures prolonged post-treatment bleeding to keep the wound clean and improve circulation.

  • Bdellins and Eglins: Act as anti-inflammatory agents by inhibiting specific proteases.

  • Hyaluronidase: Facilitates the penetration and diffusion of other medicinal substances into the tissues.

  • Neurotransmitters: Substances such as dopamine, serotonin, and acetylcholine, which contribute to pain reduction and relaxation.

Therapeutic Effects

When a leech bites, these substances are injected into the horse's bloodstream, producing several beneficial effects:

  • Anticoagulant & Thrombolytic: Thins the blood and can help dissolve existing clots.

  • Anti-inflammatory: Reduces local swelling and inflammation.

  • Analgesic: Provides a numbing effect, making the process nearly painless after the initial bite.

  • Vasodilation: Widens blood vessels to significantly improve local blood and lymph circulation.

  • Bactericidal: Possesses antibiotic properties that can help combat infections.​

Applications

Hirudotherapy is applied to a wide range of equine conditions, often with rapid and long-lasting results.

  • Tendon and Ligament Injuries: Used for tendonitis and tenosynovitis to reduce inflammation and speed up the healing of fibers.

  • Dermatological Issues: Effective for treating "mud fever" (mok) and facilitating the healing of chronic or poorly healing wounds and scars.

  • Hematomas and Edema: Helps in the rapid resolution of blood-filled swellings and fluid accumulation.
